You’ve probably seen NAD+ everywhere lately — IV drip bars, supplement aisles, wellness podcasts. It gets credited with everything from anti-aging to better energy to mental clarity. Some of that is grounded in real science. A lot of it isn’t.
Let’s walk through what NAD+ actually does, why your mitochondria need it, and — most importantly — what truly moves the needle on your energy and cellular health.
What Is NAD+, and Why Should You Care?
NAD+ (nicotinamide adenine dinucleotide) is a molecule found in every cell of your body. Think of it as a kind of cellular currency. Your mitochondria — the tiny power plants inside your cells — spend NAD+ constantly to turn the food you eat into the energy your body runs on.
Without enough NAD+, your mitochondria can’t produce energy efficiently. You feel it as fatigue, slower recovery, brain fog, and over time, the kind of decline we associate with aging.
But NAD+ does more than just fuel energy production. It’s also a signal. When your NAD+ levels are healthy, a family of proteins called sirtuins gets activated. Sirtuins are like cellular maintenance crews — they trigger your body to build new mitochondria, clean up damaged ones, and turn on antioxidant defenses. Low NAD+ means these crews stay home.
Why Levels Drop
NAD+ naturally declines with age. By your 60s, you may have roughly half the NAD+ you had in your 20s. But age isn’t the only factor — and it’s often not even the biggest one. Several lifestyle and health factors can drain NAD+ faster:
- Chronic inflammation. Inflamed tissues activate an enzyme called CD38 that breaks NAD+ down. This is one of the largest drains in the modern adult.
- Poor sleep and shift work. NAD+ follows a daily rhythm tied to your body clock. Disrupted sleep flattens that rhythm.
- Sedentary lifestyle. Exercise actually stimulates your body to make more NAD+. Sitting all day does the opposite.
- Overeating and metabolic dysfunction. Constant high-calorie intake shifts your cells into a state where NAD+ gets depleted faster than it’s replaced.
- Heavy alcohol use. Your liver burns through NAD+ rapidly to process alcohol.
- Chronic stress. Drives the same inflammatory pathways that consume NAD+.
- Excess UV exposure, smoking, and environmental toxins. All cause DNA damage that your cells repair using NAD+.
If you recognize yourself in several of these, your NAD+ levels are probably running lower than they could be — regardless of your age.
The Drip and Supplement Question
Here’s where the marketing gets ahead of the science.
IV NAD+ drips clearly raise NAD+ in your bloodstream for a while. What’s less clear is how much of that actually reaches the tissues that need it — your muscles, brain, and liver. NAD+ doesn’t easily cross into cells in its intact form. Most of what’s infused gets broken down first, then rebuilt inside cells. So an expensive drip may be doing roughly what a much cheaper oral supplement could do.
Oral NAD+ precursors — supplements like nicotinamide riboside (NR), nicotinamide mononucleotide (NMN), niacin, and niacinamide — are the building blocks your cells use to make NAD+. These reliably raise blood NAD+ levels in studies. The harder question is whether that translates into how you actually feel and function.
Human studies show mixed results. Some people, especially older adults or those with metabolic conditions, see modest benefits in energy, exercise capacity, or recovery. Others see lab numbers improve without much real-world change. The supplements aren’t magic — they’re more like topping off a tank that’s been slowly leaking.
The honest summary: NAD+ supplementation can help, especially if your levels are genuinely depleted. But it works best as a supporting player, not a starring role. If you’re inflamed, sleep-deprived, sedentary, and stressed, no amount of supplementation will outrun the leak.
What Actually Builds Better Mitochondria
The strongest, most reliable ways to improve mitochondrial health are also the least glamorous. They’re the things your body evolved to respond to:
Exercise — especially the right kinds. Zone 2 cardio (sustained moderate effort where you can still hold a conversation) builds mitochondrial density. High-intensity intervals make existing mitochondria more efficient. Strength training preserves muscle, which is the largest mitochondrial reservoir in your body. This is the single most powerful tool.
Time-restricted eating or mild caloric restriction. Giving your body a daily fasting window (12–14 hours overnight is a reasonable starting point) lets NAD+ levels recover and activates the same cellular maintenance programs that supplements try to mimic.
Quality sleep on a consistent schedule. NAD+ rhythms depend on your circadian clock. Seven to nine hours, dark room, consistent bed and wake times.
Cold and heat exposure. Cold showers, sauna sessions, or cold-water immersion all activate mitochondrial biogenesis through stress-response pathways.
Anti-inflammatory diet. Whole foods, adequate protein, plenty of vegetables and colorful plants, minimal ultra-processed foods. Reducing chronic inflammation slows the CD38-driven NAD+ drain.
Adequate B vitamins and protein. Niacin (B3), riboflavin (B2), B6, and the amino acid tryptophan are all required for your body to make NAD+ in the first place. A reasonable diet usually covers this, but it’s worth checking if you eat restrictively.
A Framework That Makes This Easier to Understand
It helps to think of mitochondrial health as a system with three inputs. All three have to be working for your cells to actually thrive. Missing any one of them, and the other two can’t fully compensate.

1. Demand — The Bill.
Your mitochondria are remarkably responsive, but they don’t upgrade themselves for no reason. They build new capacity in response to demand — situations where your body needs more energy or has to handle stress. Think of demand as a bill that comes due. Exercise, fasting windows, cold exposure, heat exposure, and physical work all create that bill. No bill, no reason for your cells to spend or invest. This is why a sedentary person with a perfect supplement stack still struggles with energy: there’s nothing forcing the cells to draw on their resources and build more capacity.
Demand is the stimulus. Nothing else matters as much.
2. Substrate — The Currency.
Once the bill arrives, your cells need currency to pay it. NAD+ is one of the main currencies of mitochondrial work, but it’s not the only one — your cells also spend oxygen, fuel (carbs and fats), B vitamins (especially B2, B3, and B6), magnesium, iron, CoQ10, and amino acids from protein. If any of these run short, your cells can’t fully pay the bill that demand has put in front of them.
Currency is where supplements actually live. NAD+ precursors, B-complex vitamins, CoQ10, magnesium — these are all attempts to make sure your cells have enough in the account when the bill comes due. They matter, but only in the context of real demand and a working response system.
3. Machinery — the response system.
This is the network of signaling proteins inside your cells — sirtuins, AMPK, PGC-1α, and others — that actually translate “the bill is due and the currency is available” into building new mitochondria, cleaning up damaged ones, and ramping up antioxidant defenses. NAD+ doesn’t just serve as currency; it also activates this machinery. That’s the dual role that makes it so important.
The machinery is trainable. It gets sharper and more responsive with regular use — every workout, fasting window, cold plunge, or sauna session trains it to respond faster and more effectively. It dulls when it’s chronically inflamed, oxidatively stressed, or simply never called on. Use-it-or-lose-it applies here as much as it does to your muscles.
How it fits together.
NAD+ sits in both the currency and machinery categories. That’s why it gets so much attention — it’s necessary in two of three places. But it’s not the bill. The drip industry sometimes sells it as if it were, which is where the marketing diverges from the biology.
Here’s the practical takeaway:
- If you have a bill but no currency (training hard on a poor diet), you’ll plateau or break down.
- If you have currency but no bill (great supplement stack, sedentary life), most of it sits in the account unused.
- If you have both but the machinery is rusty (chronic inflammation, poor sleep, never trained), the response is slow and weak.
Real mitochondrial health requires attention to all three.
Where Supplements Fit In
If you’ve already addressed the lifestyle pieces and want to add support, NAD+ precursors are reasonable. Niacin and niacinamide are inexpensive and well-studied. Nicotinamide riboside (NR) and nicotinamide mononucleotide (NMN) are newer, more expensive, and reliably raise blood levels — though the functional benefits in healthy adults remain modest in the data we have.
What I’d caution against: treating an IV drip or a bottle of supplements as a shortcut around the lifestyle work. Using the framework above, supplements only top up your currency. They don’t create a bill, and they can’t train rusty machinery on their own. NAD+ doesn’t force your mitochondria to work harder. It gives them permission to respond when there’s actually a bill due — exercise, fasting, cold, real metabolic stress. Without that, the extra currency just sits in the account.
The Bottom Line
NAD+ matters. Mitochondrial health matters even more. But the most effective path isn’t a drip schedule or a supplement stack — it’s the unglamorous work of moving your body, eating real food, sleeping well, managing stress, and giving your cells reasons to adapt.
Address the leak first. Then, if it makes sense for your situation, consider topping off the tank.
